I personally would not look at VB.net if I were building a new medical software system, but it is an option. Many of the newer EMR software vendors are building directly on web 2.0 technologies, including open source underlying technology. Take a look at the medical software vendors to gain an understanding of the technologies in use. medical software for physician offices is going almost entirely to a SaaS model in the futur.
