Please help me - this is driving me insane!!

As soon as my computer is booted up, everything takes forever. Opening "my docs" for example may take up to 5mins. Opening a program will take longer, and im at my wits end!

In the task manager i can see that the cpu usage is constantly at 100% and explorer.exe is using the lions share - 80-90%.

Any idea why this may be, and what I can do to fix it? I have tried installing AVG but im getting an error with that too! Is there any way I can install and run AVG, Spybot and Adaware without loading windows? (eg from dos or a start-up disk).

Things ive tried so far include System Restore, Starting up in Safemode (problem still exists) and also ive tried deleting a certain registry key which is a common fix for this problem ( i cant remember the exact key but its something to do with .AVI files - its listed on other posts numerously).

Please HELP!!!!
